Abstract

Introdução: A astrocitopatia por proteína glial fibrilar ácida (GFAP) autoimune foi recentemente descrita e resulta em quadro clínico de meningoencefalomielite que afeta meninges, cérebro, medula espinal e nervo óptico. Tal quadro inflamatório ocorre devido à produção de autoanticorpos para a GFAP (anti-GFAP) e recrutamento local de infiltrato inflamatório, os quais podem ser utilizados como biomarcadores específicos.

Objetivos: O trabalho visa estreitar relação entre profissionais da saúde e esta doença emergente ainda pouco relatada no Brasil, descrevendo às análises clínicas e laboratoriais, terapêuticas e à gravidade desta patologia e suas comorbidades potencialmente associadas.

Metodologia: Trata-se de uma revisão narrativa. Selecionou-se como estratégia de busca na U.S. National Library of Medicine (PubMed - NCBI) os descritores “neuroimunomodulação”, “astrócitos” e “proteína glial fibrilar ácida”, presentes nos Descritores em Ciências da Saúde (DeCS), além do nome da doença em inglês (“Autoimmune Glial Fibrillary Acidic Protein Astrocytopathy”).

Resultados: Encontrou-se 66 artigos sendo selecionados 23 que se enquadravam no escopo do trabalho.  A astrocitopatia por GFAP é uma condição rara, porém subdiagnosticada. Os principais achados clínicos são cefaleia subaguda, sintomas mielopáticos, edema do disco óptico, tremor, ataxia, rigidez do pescoço, instabilidade autonômica, confusão e comprometimento cognitivo progressivo. O diagnóstico é confirmado pela detecção de anticorpos anti-GFAP (IgG) no líquido cefalorraquidiano (LCR), estando estes diretamente relacionados a astrocitopatia e ao dano astrocítico. O tratamento no estágio agudo inclui corticosteroides endovenosos em altas doses, imunoglobulina intravenosa (IVIG) e plasmaférese. A longo prazo são utilizados esteroides orais e imunossupressores.

Conclusão: Apesar da patogênese da doença ainda não ser completamente conhecida, observamos afecção principalmente em meninges, cérebro, medula espinal e nervo óptico, estando os auto-anticorpos (Anti-GFAP) diretamente relacionados ao dano aos astrócitos podendo ser utilizados como principal biomarcador da doença, mas ela permanece em um quadro de subdiagnóstico na maioria das vezes.
