Abstract

Distração osteogênica da sínfise mandibular (DOSM) é uma terapia alternativa para tratamento de más oclusões com deficiência transversal e apinhamentos limítrofes, nos quais outras abordagens convencionais sem extração não poderiam ser realizadas pela quantidade de esmalte dentário que teria de ser removida, ou extrações seriadas que poderiam comprometer a estética pelo achatamento do perfil. A DOSM pode ser conseguida usando aparelhos dentossuportados, osseossuportados ou híbridos após a osteotomia da linha média mandibular. O objetivo deste trabalho foi apresentar um caso clínico de uma paciente do sexo feminino, 44 anos, atendida no ambulatório de Cirurgia e Traumatologia Bucomaxilofacial do Hospital Geral de Cuiabá, apresentando diagnóstico de discrepância sagital e transversal de mandíbula. Foi proposto tratamento em dois tempos cirúrgicos, sendo a priori a DOSM e, posteriormente, cirurgia ortognática. Após osteotomia da linha média e findado as ativações do aparelho disjuntor dentossuportado mandibular, concluiu-se que a DOSM é uma alternativa viável para o tratamento de más oclusões com deficiência transversal e apinhamentos, apresentando ganho significativo em perímetro do arco, estabilidade aceitável, ausências de defeitos relevantes sobre as articulações temporomandibulares e baixa morbidade.
