Abstract

A Síndrome da Realimentação é de grande relevância na prática clínica uma vez que acomete pacientes de todas as faixas etárias e em todos os níveis de assistência médica. Estes pacientes têm em comum o estresse metabólico e a depleção nutricional, condições clínicas de alta prevalência na rotina hospitalar e ambulatorial, independente da especialidade médica. As manifestações clínicas são variadas, podendo incluir náuseas, êmese e letargia até insuficiência cardíaca, arritmias, coma e morte. A conscientização dos profissionais de saúde no que diz respeito à prevenção desta síndrome aliada à abordagem multidisciplinar tendem a diminuir a morbimortalidade. A síndrome da realimentação é observada na reintrodução da alimentação após a privação de ingesta calórica (aguda ou crônica), independente da via alimentar utilizada.  É marcada laboratorialmente por distúrbios hidroeletrolíticos, principalmente hipofosfatemia, que ocorrem após o início da alimentação e que não podem ser atribuídos a outras causas. A prevenção dessa patologia é um desafio clínico, pois exige que o médico leve em consideração a avaliação nutricional do paciente, antes da prescrição da dieta, bem como tenha conhecimento dos fatores de risco que predispõem a essa síndrome. Relataremos um caso de Síndrome da Realimentação, em um paciente geriátrico, ocorrido durante internação em unidade de terapia intensiva, abordando o manejo dessa patologia, com foco voltado para a identificação de fatores de risco que podem contribuir para a sua prevenção. Uma revisão da literatura também será apresentada.
