Desfechos adversos perinatais relacionados ao consumo de álcool e tabaco na gestação: uma revisão sistemática / Perinatal adverse outcomes related to alcohol and tobacco consumption during pregnancy: a systematic review

Abstract

Analisar as complicações e desfechos adversos perinatais resultantes do consumo de álcool e tabaco durante a gestação. Pesquisou-se nas bases de dados Lilacs e Medline de 2008 até 2017. Usaram-se os descritores: Complicações na gravidez; Transtornos do Espectro Alcoólico Fetal; Intoxicação alcoólica; Tabaco. Incluiu-se artigos sobre “Obstetrícia”, com tema principal “Complicações na gravidez”, selecionou-se estudos de coorte, ensaios clínicos controlados, estudos de caso controle e relatos de caso relativos a temática abordada. Após o fim do processo de seleção, restaram 11 artigos. O tabaco é responsável por impactar em abortos, baixo peso ao nascer e outros desfechos adversos, não houve relação significativa com patologias placentárias. Além disso, o uso desta substância provou-se potencializador de complicações perinatais, dentre elas ruptura de membrana, parto prematuro e Síndrome de Morte Súbita Infantil.  Não obstante, pesquisas indicam que gestantes usuárias de narguile mais de uma vez por dia aumentavam a chance do feto nascer abaixo do peso esperado quando comparado com gestantes não fumantes. Análogo à isso, o consumo de álcool possui implicações na barreira placentária, podendo levar o feto a Síndrome Alcoólica Fetal (SAF). Ademais, o sub reconhecimento da ingestão desse precisa ser priorizada pelos profissionais de saúde que têm papel fundamental na identificação do comportamento em mulheres grávidas e mulheres em idade fértil. Ambas substâncias possuem influências negativas para o binômio materno-fetal, e, seu uso está fortemente correlacionado. Dessa forma, abster-se de tabaco e bebidas etílicas é essencial para que o período perinatal ocorra dentro dos padrões de normalidade.
