Abstract

O diabetes mellitus gestacional (DMG) é um distúrbio metabólico caracterizado pelo desenvolvimento de hiperglicemia crônica durante a gestação, em mulheres sem diagnóstico prévio de diabetes. A sua prevalência oscila a depender das populações estudadas e dos critérios diagnósticos. Em grande parte dos casos, a hiperglicemia é ocasionada por uma de resistência crônica à insulina, assim como uma redução da tolerância à glicose devido à disfunção das células β do pâncreas, havendo também diversos outros fatores relacionados. Nesse sentido, essa patologia pode ser potencializada por diversos fatores de risco como obesidade, deficiências de micronutrientes, idade materna avançada e história familiar de resistência à insulina e/ou diabetes, que são cada vez mais recorrentes. No que tange às implicações clínicas, estas são múltiplas e vinculam-se tanto ao acometimento das gestantes, quanto de sua prole, como risco de desenvolvimento de quadros hipertensivos e a macrossomia, respectivamente. Dentro desse contexto, para além dos múltiplos fatores de risco já conhecidos, observou-se, no âmbito da pandemia do Coronavírus, um paralelo entre a presença do DMG e uma maior probabilidade de gravidade na infecção pelo vírus da Covid-19, direcionando a uma maior atenção para essas pacientes. Diante dessas observações, a identificação da DMG relacionada, ou não, ao contexto da pandemia da Covid-19, abordagem clínica apropriados desse acometimento, bem como um manejo terapêutico direcionado influenciarão positivamente em seu prognóstico.
